Our History
Rooted in Collaboration Since 1997
North Country Health Consortium was founded in 1997 when four organizations came together around a shared idea: that many of the health challenges facing northern New Hampshire could be addressed more effectively by working together.
What began as the North Country Health Information Network has grown and evolved alongside the communities we serve. As new needs have emerged, NCHC has continued to bring together partners, pursue resources, and develop programs and services designed for the realities of rural northern New Hampshire.
Over the years, our work has expanded across public health, healthcare workforce development, prevention and recovery, youth engagement, community health, and support for individuals and families. Programs and initiatives have grown, changed, and sometimes concluded as community needs and funding have evolved—but NCHC's role has remained remarkably consistent: bringing people and organizations together to identify challenges, build solutions, and strengthen the health of our communities.
Nearly three decades later, collaboration remains at the heart of who we are. Our history is one of listening, adapting, and working together—and we continue to evolve alongside the communities we are proud to serve.
Our Journey
1997
North Country Health Consortium is formed when four organizations come together to establish the North Country Health Information Network.
1999
The Northern New Hampshire Area Health Education Center (NNH AHEC) is established, beginning a longstanding commitment to strengthening the rural healthcare workforce.
Early 2000s
NCHC expands its work in regional public health infrastructure, substance misuse prevention, community health, healthcare access, and care coordination.
2010s
NCHC continues to grow its role as a regional convener and partner, supporting initiatives spanning prevention, youth, workforce development, public health, behavioral health, oral health, and community-based services.
2020 & Beyond
NCHC continues to respond to changing community needs, building new partnerships and initiatives while strengthening the systems, people, and communities that support health across northern New Hampshire.
Still Growing. Still Collaborating.
The programs and initiatives of NCHC will continue to evolve, just as they have since 1997. What remains constant is our commitment to bringing people together and finding collaborative solutions that improve health throughout northern New Hampshire.
