Supporting the Continuum of Care for Substance Use

What is the Continuum of Care?

The Continuum of Care (CoC) is a framework that encourages communities to support resiliency and recovery and to lay the foundation for public health regions to begin to address the gaps in the continuum over time. In coordination with this approach, the NH Bureau of Drug and Alcohol Services (BDAS) is committed to developing comprehensive, effective, and accessible continuum of care in every region of the state that includes prevention, identification, and early intervention, treatment, and recovery supports, and is integrated with primary and behavioral health.

 

 

Integration of CoC in the North Country

Partnering primary care and behavioral health services to provide a full spectrum of quality prevention, intervention, treatment, and recovery support services, the North Country Continuum of Care initiative is designed to comprehensively address substance misuse in the region.

The North Country Public Health Network, including the Substance Misuse Prevention (SMP) Network, is committed to identifying and coordinating a robust and effective Continuum of Care in Northern New Hampshire, including: Prevention, Intervention, Treatment, and Recovery.
